The Arizona Department of Human Services (DHS) has recently announced updates to its official DHS Record Card format. These changes, while minor, will impact all Arizona programs that use the standard Arizona DHS Record Card to meet licensing requirements. Arux has made changes alongside Arizona DHS to keep our customers in compliance.
What's changing
The updates to the Arizona DHS Record Card include:
New Arizona DHS logo: The updated card features the refreshed Arizona DHS branding.
Enhanced contact information: Email addresses have been added to both Parent and Guardian contact sections, providing an additional communication method. Additionally, outdated and typically unused fields were removed for streamlining the form.
Improved text and formatting: Various text elements and formatting have been updated throughout the document for better clarity and readability.
Updated footer date: The date in the document footer has been updated to reflect the current version.

What this means for Arizona programs
All districts and programs operating in Arizona will need to implement these changes to ensure compliance with DHS requirements. Programs should update their enrollment questions and documentation to match the new format specifications.
